All chits are either Word (.doc) or Excel (.xls) files that have been properly formatted in accordance with USNA instructions. Personal, chain of command, and other variant information should be entered into the spaces marked with either red text (Word) or cells with red comment triangles (Excel). Examples of proper formatting for each space are shown or can be view by dragging the cursor over each marked space. These formatting notes can be overwritten by clicking on, and typing over their fields.

Plebe Church Liberty Special Request Chit (.xls)

Download, print, and sign 3 (three) copies of this chit to receive special limited town liberty on Sundays, enabling you to attend Sacrament Services with the Annapolis Ward (Midshipmen attend the Annapolis Ward's services every Sunday from 0900-1300. Rides are provided for all midshipmen and guests of the Naval Academy, departing from the parking lot nearest Dahlgren Hall's Drydock restaurant and the Superintendent's House).

This special request chit can be downloaded and used for any request. The remarks and reasons sections have been left blank, with examples for proper formatting provided. 

Memorandum (.doc)

This is a memorandum formatted to be routed up through a company's chain-of-command.  Enter your personal information in the fields marked with brackets and red text by clicking on and overwriting the example shown in red text. 

Counseling Sheet (.doc)

A template for initial and periodic Midshipmen counseling.  Personal information can be added in the fields marked with brackets and red text by clicking on and overwriting the example shown in red text. Boxes can be marked by double-clicking and changing the default value.
