Application Instructions

Participation in the Sponsor Program allows people living within the U.S. Naval Academy vicinity to sponsor incoming Midshipmen. Acceptance into this Program does not guarantee you will be assigned a Midshipman. The information you provide on the application will be used to determine your eligibility for the Program and to find the best possible match between you and up to four incoming Midshipmen (Plebes). Stars indicate required items.

• Personal Information: This section asks for information about you.
• Title: Select a response from the pull-down list.
• *First Name: Enter your first name. If you prefer to be addressed by another name, enter it in the Preferred Name item below.
• Preferred Name: If you prefer to be addressed by a name other than your first name, enter it here. Otherwise, leave this item blank.
• *Last Name: Enter your last name.
• Name Suffix: Select a response from the pull-down list. If you do not have a name suffix, leave this blank.
• *Gender: Select a response from the pull-down list.
• *Birth Date: Enter your birth date in the format DD-MON-YYYY (for example, 06-AUG-1959). You may use the pop-up calendar to enter this information.
• *Marital Status: Select a response from the pull-down list.
• *Home Address: Enter your home address. Only the first line is required.
• *City: Enter the city for your home address.
• *State: Enter the state for your home address.
• *Zip Code: Enter the zip code for your home address. You must live within 22 miles of Annapolis to be eligible for the Sponsor Program.
• *Email Address: Enter your email address. The Sponsor Program Coordinator will use this address for important communications. Please enter only one address.
• *Home Phone Number: Enter your home phone number including area code. The number may be entered with or without dashes.
• Work Phone Number: Enter your work phone number including area code. The number may be entered with or without dashes.
• Cell Phone Number: Enter your cell phone number including area code. The number may be entered with or without dashes.
• Military Branch and Rank/Rate: If you are active duty, reserve, or retired military, click on the link and select the appropriate service branch and rank/rate from the list in the popup window. The corresponding values for both Military Branch and Rank/Rate will be entered on the form.
• Military Status: Select a response from the pull-down list. This is required information if you are a member of the U.S. Armed Forces.
• Current Employer: If you are currently employed, provide the name of your employer.
• Employer Address: Enter the full address of your employer.
• Employer Phone Number: Enter your employer’s phone number, including area code. The number may be entered with or without dashes.
• Occupation: Enter your job title at your place of employment.
• *Sponsor Status: Select a response from the pull-down list.
• *Can you speak a language besides English?: Select "Yes" or "No" from the pull-down list.
• *If Yes, please indicate the language. If you speak more than one or if you speak a language that is not listed, select "Multiple" or "Other" and specify the name of the language in Additional Comments.: If you answered "Yes" to the previous question, select a response from the pull-down list.
• *Have you ever been convicted of a felony, domestic violence, drug-related offense, sex offense, DWI/DUI, or had your driver’s license revoked?: Select "Yes" or "No" from the pull-down menu.
• If yes, give a complete description of the incident(s). State where and when each incident occurred, the nature of the offense(s) and the date and disposition of the case(s): If you answered "Yes" to the previous question, provide all the information requested. Otherwise, leave this entry blank.
• *Provide a Statement of Interest as to why you would like to be a part of the Sponsor Program and what you feel you have to offer: Explain why you are applying for the Sponsor Program and why you would be a good sponsor.
• Spouse Information: This section asks for information about your spouse, if you are married.
• Spouse First Name: Enter your spouse’s first name. If your spouse prefers to be addressed by another name, enter it in the Preferred Name item below.
• Spouse Preferred Name: If your spouse prefers to be addressed by a name other than his or her first name, enter it here. Otherwise, leave this item blank.
• Spouse Last Name: Enter your spouse’s last name.
• Spouse Birth Date: Enter your spouse’s birth date in the format DD-MON-YYYY (for example, 06-AUG-1959). You may use the pop-up calendar to enter this information.
• Spouse Employer: If your spouse is currently employed, provide the name of your spouse’s employer.
• Spouse Employer Address: Enter the full address of your spouse’s employer.
• Spouse Employer Phone Number: Enter the phone number of your spouse’s employer, including area code. The number may be entered with or without dashes.
• Spouse Occupation: Enter your spouse’s job title at his or her place of employment.
• Has your spouse ever been convicted of a felony, domestic violence, drug-related offense, sex offense, DWI/DUI, or had his or her driver’s license revoked?: Select "Yes" or "No" from the pull-down menu.

• Household and General Midshipman Preferences: The information requested in this section will be compared with corresponding information obtained from incoming Midshipmen to try to match you with the most compatible Midshipmen.
• *Children: Select a response from the pull-down list.
• *Pets: Select a response from the pull-down list. If you have pets that are not listed, select "Other," and describe these pets in the "Additional Comments or Special Considerations" item at the end of the form.
• *Allow Smoking: Select a response from the pull-down list.
• *Number of Midshipmen You Wish to Sponsor (limited to four per class year): Select a response from the pull-down list.
• *Midshipman Gender: Select a response from the pull-down list.
• *Midshipman Home State: Select a response from the pull-down list.
• *Midshipman Military Background: Select a response from the pull-down list.
• *Do you prefer a non-smoker?: Select "Yes" or "No" from the pull-down list.
• *Midshipman Varsity Sports Affiliation: Select a response from the pull-down list.
• Indicate Your Top Interests From the Following Five Categories: Select up to five interests from the categories of Sports, Outdoor Activities, Crafts/Hobbies, Music and Other.
• *Preference Priority 1, 2, 3, 4, and 5: Using the values in the pull-down lists, assign Priority 1 to the area you consider most important, Priority 2 to the next most important, and so on through Priority 5 as the area you consider least important. Each of the values should appear once. These items allow the Sponsor Program Coordinator to weight the consideration given to different areas covered in the preceding items when matching you with incoming midshipmen.
• Specific Midshipman Request: If there are specific incoming Midshipmen whom you wish to sponsor, fill in the information requested in this section. The Midshipmen whom you request must be aware that you are requesting him/her. The Midshipmen must also request you as a sponsor in order for the request to be honored. You may request up to four specific Midshipmen.
• Last Name: Enter the last name of each incoming Midshipman you would like to sponsor.
• First Name: Enter the first name of each incoming Midshipman you would like to sponsor.
• Home State: Enter the home state of each incoming Midshipman you would like to sponsor.
• Is the Midshipman Aware of Your Request?: Select "Yes" or "No" from the pull-down list.
• Gate and Vehicle Pass Information: The information provided here will be used for a FONA (Friends of the Naval Academy) pass. You may list up to two vehicles.
• Year: Enter the model year of each vehicle.
• Make: Enter the make or manufacturer of each vehicle. (Ford, Toyota, Audi, etc.)
• Model: Enter the model of each vehicle. (Taurus, Tacoma, Quattro, etc.)
• Color: Enter the color of each vehicle.
• State Registered: Enter the name of the state in which each vehicle is registered.
• License Plate Number: Enter the license plate or tag number of each vehicle.
• Training Information: Sponsor training is required for new Sponsors and every three years for renewing Sponsors. The dates for training sessions can be accessed from the Sponsor Program web site. Select "Yes" for only one of the sessions.
• *Session One: Select "Yes" from the pull-down menu if you intend to attend sesson one; otherwise select "No."
• *Session Two: Select "Yes" from the pull-down menu if you intend to attend sesson two; otherwise select "No."
• *Session Three: Select "Yes" from the pull-down menu if you intend to attend sesson three; otherwise select "No."
• Additional Comments or Special Considerations: If you answered "Other" to any questions on the application, or you would like to provide additional information to the Sponsor Program Coordinator, please enter the information here.
