High School Transcript Application
Year of High School Graduation: Enter the four-digit year in which the candidate will or did graduate from high school.
Cumulative Grade Point Average: Enter the candidate's grade point average (GPA) using the format #.##. There can only be one digit preceding the decimal point, and the limit is two decimal places. If your school uses a 0-100 percent scale, please convert to a GPA.
Class Rank/ Percentile: Enter the candidate's numerical standing in his or her class or the percentile occupied (Do not provide both). If your school does not rank students, please approximate a percentile based on academic performance among peers. Next, enter the total number of students in the candidate's class. Indicate if the provided class rank is exact or an approximation. Please provide starting and ending time period of rank calculation in the format MON-YYYY, where MON is the first three letters of the month and YYYY is the four-digit year. Finally, if a school profile will not be sent with the student's transcript, please provide information about how class rank is determined.
Percent 2/4-Year College: Enter the percentages of students in the graduating class expected to attend both 2-year and 4-year colleges and universities. A number between 0 and 100 must be entered for each.
Answer the remaining questions with either YES or NO using the pull-down menus.
Remarks: Please provide the following information:
(1) Additional information which may be significant in considering candidate concerning achievements or unique circumstances.
(2) Has the candidate ever been subjected to discipline (probation, suspension, or expulsion) at your school? If so, please explain.
(3) Has the candidate required any special accommodations, or been assigned an Individual Education Plan for assistance with an Academic Skills Disorder, ADHD, or dyslexia?
In addition, please feel free to elaborate on the candidate's performance in your school or in non-academic pursuits. This field is limited to 3,000 characters. If you would like to include a separate written statement, please mail it to:
On all sent documents, be sure to include the student's Candidate Number which can be found at the top of the application.
Contact Information: Your high school name and location are pre-entered in this form. If you see a discrepancy, please contact the Admissions office at (410) 293-1858 or by email. You may submit this form with an incorrect school or location listed, and the information may be corrected after submission.
Finally, provide a current phone number and list your official job title. Upon submitting this form successfully, you and the candidate will receive notification of receipt by the Naval Academy.
Please attach a digital transcript file to the bottom of the form in the section titled 'Attachment'.
For current high school students, please upload an official transcript containing all grades 9th through 11th grade. For students who have already graduated from high school, please upload complete high school transcript through 12th grade.
If it is not possible to attach a digital copy of the transcript, then after submission, please send a sealed official transcript to:
