Revising an Official USNA Form

  1. Initiate a DD 67 Form Action Processing Request.  Ensure blocks 1-12, 14, and 17a-c are filled out before submitting the form. Send the DD 67, current version of the official form, summary of changes you would like made to the form, link to prescribing issuance, and any other documents to forms@usna.edu to initiate the revision. If the form is not prescribed in a directive, a draft directive prescribing the form must be submitted with the change request.  

  2. Once received, the USNA Forms Office will review the package for accuracy and reach out to sponsors if additional information is needed. Additional items that may be required can include: SECNAV 5213/1  SSN Reduction Review if the change adds the use of a full or partial SSN, an OMB Submission Package if the form change includes more than 10 respondents that are members of the public each year, or a System of Records submission package if the change incorporates saving the information in a new System of Records.

  3. The USNA Forms office will make the changes to the official forms utilizing the current Navy Forms Design software and adhering to the DoD design standards.  If the design standards can not be utilized, the USNA Forms Office can waive part or all design standards.  If waiving part or all of the design standards, this will be annotated on the DD 67.  Only the Forms Office can waive the design standards for pdf forms created for use by USNA.  As a note, forms created for use completely within the USNA AEEE system do not have to follow the design standards, but all other information requirements must be meet and the form must go through the same review and approval process as pdf forms.

  4. Once the changes have been made, the USNA Forms office will send the draft form back to the sponsor for testing and approval.

  5. Once the revision is approved and all other paperwork complete, the forms office will post the revised form to the USNA Official Forms Website.
